File a Claim - Extended Warranty Plans

Please read below details carefully before filing your claim. File your claim within 48 hours of the incident.

*In case of Hospitalization of customer due to any mishap its 60 days to file a claim.

*Or any other issue/complaint then E-Mail us on:

If your product fails to operate due to a covered reason, follow these steps to file a claim.

Before filing claim, ensure the following:
• Product return policy period on the product is expired.
• Your
PROINDEM Product Protection Plan is in effect.
​

Be sure to have the following with you:
• Product Purchase Proof (receipt, Invoice etc.)
• PROINDEM Product Protection Plan Certificate/ Customer Identification Number.

• Covered product.

If Product Protection Plan was purchased and Registered/Activated within 15 Days of New Product/Device Purchased and Eligible for Insurance Claim, We may ask you below Documents:

 

Damaged Handsets:

  1. Photo copy of bill

  2. Customer’s Id proof-self attested

  3. Insurance form (Claim form)

  4. Declaration letter-self attested

  5. 3 photos of damaged Handset (One with the IMEI number)

 

Theft Handsets:

  1. Original bill copy

  2. FIR (First Information Report from Incident Pin code Related Police Station only, to be produced within 48hrs of the incident)

  3. SIM blocking letter (Form 3A)

  4. Declaration letter-self attested

  5. Insurance form (Claim form)

  6. Subrogation form

  7. ID proof

 

Dead on arrival (DOA) Cases:

  1. New bill

  2. Old bill

  3. DOA letter/Replacement letter

 

Medical Certificate (Only from MBBS or Relevant Specialist Doctor):

  • Required for all accident cases.

  • Required for all Beaten up or Hit & Run cases

 

Note:If the customer purchases any of the covered brand of phones online, we will require his product delivery invoice/email later during the documentation process of his claim.

After filing Claim if Product repairing is not possible to repair at Home/Doorstep service you will have to walk-in at our nearest service center.

Or if service center not available in your Town, we provide Pick-up and drop service by our end.

• For Electronics and Appliance Protection Plan: Arrange for In-Home, Carry-In or shipping service depending on the size of the product and the nature of the failure.
• For Handset Protection Plan: Arrange for Carry-In service at one of our authorized repair centers near you or arrange for a shipping service.
• For Laptop & Desktop Protection Plan: Arrange for Carry-In service at one of our authorized repair centers near you.

When we arrange for pickup service kindly be ensure to cross-check pickup executive ID proof as well challan matches with details of pickup executive provided to you on your "Email" and "SMS" by us.

Note: Theft/Lost Claim are processed and approved/disapproved at the sole discretion of Insurance Company. Whereas, All other claims are processed by PROINDEM.
